Enrolment

As at 28 March 2022 our books are closed for enrolling new patients.

 

Enrolling is free and voluntary. If you choose not to enrol you can still receive health services from High Street City Health.
Advantages of enrolling are that your visits to the Doctor will be cheaper and you will have direct access to a range
of services linked to High Street City Health.

 

How do I enrol?
To enrol, you need to complete an Enrolment Form. Parents can enrol children under 16 years of age but young people
over 16 years need to sign their own form.

 

Our records will show whether or not you are enrolled - if you're not sure, ask one of our team. Some of our paper records were lost in the Canterbury earthquakes so if you haven't been in to see us for a while, we may ask you to complete another form to replace one you have previously filled out.

 

If you would like to enrol with High Street City Health, please do so in person at 62 Riccarton Road.
